How to use this checklist: Click any section heading to expand it. Complete any part that applies to you and simply move on from anything that does not.

We understand this form is long, but our goal is to make sure we capture all of your qualified expenses so you receive every deduction you are legally entitled to. Attach or upload copies of your slips and receipts when you book your appointment (PDFs or clear photos are fine).

Your responsibility: It is your responsibility to keep all original receipts and supporting documentation for the expenses claimed. You do not need to send them with the return, but the Canada Revenue Agency may request them at any time and they should be retained for at least six years.

Complete this section only if your employer required you to pay employment-related expenses and provided you with a signed declaration form. Includes home office for employees, commission salespersons, and tradespersons.

Complete if you used a personal vehicle to earn business or employment income. A logbook of business kilometres is strongly recommended — the CRA expects one.
Complete if you used part of your home to earn business or self-employment income. (Employees with a signed employer declaration — use Section 6 instead.)
Include only amounts NOT covered or reimbursed by insurance. You can claim for yourself, your spouse, and your dependent children. The best 12-month period ending in 2024 may be used. Generally worth claiming if total exceeds $2,835.
Please provide receipts that include the caregiver's name, address, and SIN (if an individual). Annual caps: $8,000 per child under 7, $5,000 per child age 7–16, $11,000 if eligible for the Disability Tax Credit.
Provide your tuition slip for yourself or any dependant who attended a post-secondary institution.

A copy of the written agreement or court order is required. Spousal support is deductible to the payer / taxable to the recipient; child support is not.

Deductible only if you moved at least 40 km closer to a new work location, business, or post-secondary school. Keep all receipts.
These disclosures carry steep penalties if missed — please confirm even when answering "no".
Bankruptcy / insolvency disclosure
I confirm that the information provided above is complete and accurate to the best of my knowledge. I will retain all supporting documentation in case it is requested by the Canada Revenue Agency.
